YOU OBTAIN

A diabetic foot exam from your doctor.  Bring this form to your doctors visit and have him complete the second page to ensure proper documentation is collected

The doctors foot exam and notes (printed form) to us at 866-270-9540

WE SCHEDULE

Your shoe fitting and consultation. After this you can pick any style of shoe from Dr. Comfort. Insurance covers all styles EXCEPT slippers and sandals.

IT TAKES

Approximately 3-4 weeks from start to finish.  This time period begins once we receive your paperwork. Generally it takes 1-2 weeks for your consultation and an additional 1-2 weeks for your shoes to arrive
